Introduction

The Fertility Treatment Rebate-2 is designed to help with the costs of IVF and other Assisted Reproductive Technology (ART) treatments.

Eligible women in NSW who have received an eligible fertility treatment on or after 19 February 2025, can apply for the $2,000 rebate.

Note: You must apply within 90 days of the date you received the ART treatment.

You can only get the rebate once.
You cannot apply for the Fertility Treatment Rebate-2 Program rebate if you received a rebate for fertility treatment you had between 1 October 2022 and 18 February 2025.

Eligibility

To apply you must be a woman who:

  • is a resident of NSW
  • has incurred eligible costs for fertility treatment(s) received on or after 19 February 2025
  • meets one of the following household income eligibility criteria:
    • receiving Family Tax Benefit
    • having a Health Care Card
    • having a household income of $116,000 or less
  • has received fertility treatment from an eligible ART provider who:
    • is certified to deliver ART treatments by the Reproductive Technology Accreditation Committee (RTAC) as per the RTAC Australia and New Zealand Code of Practice, and
    • is authorised to operate in Australia.

Note: Service NSW acknowledges people in the transgender community, or who are non-binary, may receive fertility treatment but may not identify as women.

Ineligible costs

You cannot claim out-of-pocket costs that have been reimbursed by another government program or rebate, such as:

  • Medicare
  • the fertility treatment rebate for treatment received from 1 October 2022 to 18 February 2025.

You must not have received the treatment:

  • at a publicly supported lower cost IVF clinic as part of the NSW Affordable IVF Program
  • for the sole purpose of fertility preservation.

What you need

  • a MyServiceNSW Account
    • you can create one when you start your application
    • you may need details from 2 identity documents (such as Australian driver licence, Medicare card, passport)
  • the Fertility Treatment Rebate-2 Declaration Form – PDF completed by the provider and yourself
  • proof of your residential address in NSW, such as one of the following documents in your name:
    • most recent utility bill
    • current residential tenancy agreement
    • a screenshot from Rental Bonds Online showing details of your current residential tenancy agreement
    • other proof if you cannot provide a utility bill or tenancy agreement
      • a council rates notice
      • home insurance documents
  • proof of eligibility, either
    • Centrelink Customer Reference Number (CRN) or
    • proof of household income:
      • your ATO Notice of Assessment for the financial year 2023-24 or 2022-23 (whichever is the most recent)
      • if applicable, your partner or spouse's ATO Notice of Assessment for the same financial year
        Note: you will need to confirm that your partner or spouse has authorised you to provide their personal information to Service NSW for the purposes of the rebate
  • your bank account details (required for payment if your rebate application is successful)
  • proof of payment of treatment(s), showing:
    • your full name
    • a description of the treatment you received
    • the date of your treatment
    • the out-of-pocket cost (of each treatment claimed)
    • the name of your fertility specialist
    • the name of your provider.

Note: You can claim the full $2,000 rebate even if your eligible costs are less than $2,000. 

How to apply

  1. Check the eligibility requirements.
  2. Gather your supporting documents.

  3. Select the ‘Apply online’ button.
  4. Log in, or create your MyServiceNSW Account.
  5. Follow the prompts to apply for the rebate.

More information

  • You can only claim the rebate for out-of-pocket costs after you have made any Medicare claims.
  • If you received the Pre-IVF Fertility Testing Rebate, you can still apply for the Fertility Treatment Rebate-2.
  • The rebate is available until 30 August 2028, or until the available funds are exhausted.
  • Check the list of current registered ART providers in NSW.
