Introduction
If you want to update your date of birth or gender on your driver licence, NSW Photo Card or Mobility Parking Scheme permit, you need to visit a Service NSW Centre.
What you need
- to correct the date of birth, a List 1 proof of identity document showing your correct date of birth
- for change of gender, one of the following:
- birth certificate showing your gender
- List 1 proof of identity document showing your gender
- a Recognised Details Certificate issued by NSW Registry of Births, Deaths and Marriages (or equivalent Australian authority) stating your gender
- a medical certificate confirming your gender from an Australian registered medical practitioner
- a medical certificate confirming your gender from the surgeon or clinic where the gender reassignment surgery was conducted
- if you need a replacement product such as driver licence card, the relevant form:
- Change of Records – PDF if you do not need a replacement product.
Note: Original paper documents that have been laminated are not accepted as proof of identity.
How to change
- Gather the required documents.
- Visit a service centre and submit your application.
