Request a copy of your NSW Driver Licence photo

Introduction

If you'd like a copy of your stored NSW Driver Licence photo from Transport for NSW, please visit a Service NSW Centre.

What you need

• your current NSW Driver Licence or Photo Card
• the PDF form – 'Access to Own Personal Records'.

How to request

1. Select the 'Download PDF form' button.
2. Complete the form.
3. Visit a service centre and submit your request in person.

More information

• Once the request is processed, a hard copy of your photo will be sent to the service service.
• You need to collect your photo in person, at the service centre.
