Introduction
If your food safety supervisor certificate has been lost or damaged, contact the registered training organisation (RTO) that issued the certificate for a replacement.
If the RTO is no longer operating or has let its NSW Food Authority approval lapse, you can lodge an application with the Food Authority. They will be able to reissue a replacement certificate.
Eligibility
You must have completed:
- all the required units as indicated on the form
- a qualification at a registered training organisation (RTO) in Australia
- training within 5 years from the date of request.
What you need
- your personal details
- your RTO's name and location
- a certified copy of your certificate and transcript, or
- certified copies of your statement/s of attainment
- your payment (as indicated on the form)
- the PDF form – 'FSS certificate request form'.
Note: A ‘certified copy’ is a photocopy which has been compared with the original and endorsed as a true copy by a Justice of the Peace (JP) or a public officer such as a police officer.
How to apply
- Check the eligibility requirements.
- Select the 'Download PDF form' button.
- Complete and sign the form.
- Submit your application by mail, with payment, to the details on the form.
More information
- Your FSS certificate is valid for 5 years from the date of issue, after that it will need to be renewed
- If your FSS certificate has expired or is about to expire, you need to recertify with an approved RTO. You will have 90 days to replace the expired certificate.
