Replace an unregistered vehicle permit (UVP)

Introduction

If you hold an unregistered vehicle permit (UVP) and your permit is lost, destroyed or stolen, but is still current, you can apply for a new one at a Service NSW Centre.

A replacement fee will be charged.

What you need

• your proof of identity
• the PDF form – 'Replacement Application'.

How to replace

1. Select the 'Download PDF form' button.
2. Complete the form.
3. Make sure you have the required documentation.
4. Visit a service centre and provide the information in person.
