Apply to replace a pesticide licence identification card

Introduction

If your pesticide licence identification card has been lost, stolen, damaged or destroyed, you can apply for a replacement licence by completing an online form.

The form can also be used to request a replacement licence if you did not receive your licence or if it had a printing error.

Eligibility

Holders of the following licences:

• Pest Management Technician (PMT)
• Ground Applicator (GRN)
• Fumigator (FUM)
• Timber Pest Management Technician (PMT-TP).

What you need

• your current licence (if you have it)
• one passport-size photo
• your payment, as indicated on the form (if applicable).

How to replace

1. Select the ‘Apply online’ button.
2. Complete the form.
3. Select 'Sign and submit'.
