Introduction
If your child was born in NSW, and you want to add the biological father's details to the child's birth certificate, you can lodge an application at a service centre, or post the required documentation to the Registry.
Eligibility
- the birth must be registered in NSW
- both biological parents must sign the form, or a court order or a parentage testing DNA report may be required, to determine paternity.
What you need
- the personal and contact details of the mother
- the personal and contact details of the father
- the child's personal details
- 3 certified forms of identification for each parent
- original copies of birth certificates or extracts previously issued by the Registry
- marriage details (if applicable)
- your payment
- the PDF form – 'Adding a parent's details to a birth registration'.
How to apply
- Check the eligibility requirements.
- Select the 'Download PDF form' button.
- Complete the form.
- Make sure you have all the required documentation.
- Lodge the information using the details on the form.
More information
- Make sure you include the original copies of birth certificates or extracts previously issued by the Registry, when you lodge your application.
- Certificates are sent by the Registry of Births, Deaths & Marriages via registered post. They can't be collected at a service centre.
- Turnaround times commence when applications are received in full compliance with Registry requirements, and exclude delivery time.
- If you change your mind, your application fee will not be refunded.
- Documents provided as proof of identity may have their authenticity verified through the National Document Verification Service (DVS).
