Apply for a vehicle registration as the carer or guardian of the registered operator

Introduction

To apply to register a vehicle as the carer or guardian on behalf of the registered operator, you'll need to visit a Service NSW Centre.

What you need

• the PDF form – 'Application for Registration' in the applicant's name (the carer or guardian)
• proof of identity of the carer or guardian
• proof of carer or guardian status (court order, Power of Attorney or statutory declaration)
• proof of registration entitlement in the name of the operator or the carer or guardian
• a pensioner concession card in the name of the registered operator 
• an Authorised Unregistered Vehicle Inspection Scheme (AUVIS) report (blue slip)
• proof of compulsory third party (CTP) insurance.

How to apply

1. Select the 'Download PDF form' button.
2. Download and complete the form in the applicant's name (the carer or guardian).
3. Gather the required documents.
4. Visit a service centre and submit the application.
