Introduction
To apply to register a vehicle as the carer or guardian on behalf of the registered operator, you'll need to visit a Service NSW Centre.
What you need
- the PDF form – 'Application for Registration' in the applicant's name (the carer or guardian)
- proof of identity of the carer or guardian
- proof of carer or guardian status (court order, Power of Attorney or statutory declaration)
- proof of registration entitlement in the name of the operator or the carer or guardian
- a pensioner concession card in the name of the registered operator
- an Authorised Unregistered Vehicle Inspection Scheme (AUVIS) report (blue slip)
- proof of compulsory third party (CTP) insurance.
How to apply
- Select the 'Download PDF form' button.
- Download and complete the form in the applicant's name (the carer or guardian).
- Gather the required documents.
- Visit a service centre and submit the application.
