Introduction
If you wish to supply drugs of addiction at your health facility, for example at a licensed nursing home or private health facility, you'll need to apply for a licence.
Eligibility
An individual or corporation is eligible if they can show the Secretary of NSW Health that:
- they're fit and proper to be a licensee, and
- the premises are appropriate for the supply of drugs of addiction.
What you need
- the PDF form – Application for a Licence to Supply Drugs of Addiction
- the PDF form – Statutory Declaration
- the PDF form – Fitness and Probity Check
- details of the applicant and contact person
- facility's name and address
- full company extract from the Australian Securities and Investment Commission (ASIC), if applicable
- copy of the certificate of incorporation (if applicable)
- copy of the Act incorporating the church or organisation (if applicable)
- copy of the charitable institution status from the Australian Taxation Office (if applicable)
- a National Police Check
- ASIC certificate of registered business name
- details of the uses to be made of the drugs of addiction
- your payment.
How to apply
- Select the 'Download PDF form' button.
- Complete and sign the form.
- Gather all the required documents.
- Make your payment online at the NSW Health Payment Portal.
- Send the completed application form, the required documents and a copy of the payment receipt to the email on the form.
