Introduction
If you're the biological parent, you can apply to have your details added to your child's pre-adoptive birth registration, providing the child's birth was registered in NSW.
You'll need to provide the reason(s) why the details were not included at the time of the child's birth.
Eligibility
You need to be the biological parent to apply to be added to your child's pre-adoptive birth certificate.
Note: In some circumstances, if the child makes the request, the NSW Registry of Births, Deaths and Marriages is able to add the biological parent's details.
What you need
- 3 proof of identity documents as indicated on the form
- your payment
- the PDF form – 'Adding a parent’s details to a pre adoptive birth registration'. A statutory declaration is included in the form.
- an authorised witness, as indicated on the form, to witness your declaration.
Note: In some circumstances, the NSW Registry of Births Deaths and Marriages may require additional documentation such as:
- a parentage testing DNA report or a court order to confirm paternity
- documentation from the time of the adoption.
How to apply

- Complete the form.
- Gather your supporting documents.
- Lodge your application in person at a Service NSW Centre or post it to the details on the form.
