Service NSW and the Ministry of Health are delivering the Fertility Treatment Rebate Program (the Program) on behalf of the NSW Government. Service NSW is collecting your personal and health information to assess your eligibility for the Program, to facilitate your participation in the Program and for related purposes.
Service NSW gives priority to protecting the privacy of your personal and health information. We do this by handling personal information in a responsible manner and in accordance with the Privacy and Personal Information Protection Act 1998 and Health Records and Information Privacy Act 2002.
Why we are collecting your personal information
Service NSW is collecting your personal information for the purposes of:
- processing your application and determining your eligibility to receive a rebate and participate in the Program,
- providing you with further information about the Program
- contacting you to communicate about your application or participation in the Program
- responding to and communicating about complaints and enquiries in relation to the Program
- establishing or maintain records in your MyServiceNSW Account profile
- other directly related purposes.
What personal information we collect
Service NSW may collect the following personal and health information:
- name
- date of birth
- contact details such as email address, phone number and residential address
- identity document details such as drivers licence and Medicare card
- banking details (including bank account name, BSB and account number)
- information to satisfy NSW residency requirements such as copy of a utility bill, lease agreement etc.
- proof of name change (where applicable) such as Marriage certificate or BDM issued Change of Name certificate
- completed Fertility Treatment Rebate Declaration Form
- invoices or receipts indicating out of pockets expenses
- name of the specialist
- name of the clinic
- clinic address
- name of the clinic representative
- RTAC Accredited Unit Number
We may also ask you to provide additional information or documentation about your application that may include personal information.
How we will use and disclose this information
Service NSW may use your personal information to:
- verify the information you provide with a public/private authority
- review your application to determine the eligibility for a rebate
- conduct auditing, investigation and compliance activities to assess whether the eligibility criteria and Terms and Conditions have been and are being met, to investigate any allegations of fraud or failure to comply with Terms and Conditions and to assess the financial integrity of the Program
- contact you about your application, including to confirm the information in your application or to request additional information or to discuss the outcome of your application
- collect, maintain and use information about you for our internal administrative purposes, including for the purpose of our interactions with you.
Service NSW maintains and uses information collected about you for internal administrative purposes, including for the purposes of our interactions with you. Personal information will be used for the purposes for which it was collected or a directly related purpose. It may also be used and disclosed where we are authorised or required to do so by law.
Service NSW may disclose the information in your application to:
- public/private entities (including Document Verification Service and relevant clinics) in order to verify the information you provide in support of your application or request additional information
- NSW Police, if Service NSW reasonably suspects a case of fraud or other criminal activity
- share de-identified information for research and statistical purposes in accordance with the Data Sharing (Government Sector) Act 2015.
Service NSW will not disclose your personal information to anyone else unless you provide your consent, or we are required or authorised to do so by law. This may include disclosure to other government agencies and partners. Disclosure may also be made to law enforcement, dispute resolution, statutory or regulatory bodies.
Are you required by law to provide us with this information?
Service NSW will only collect personal information that is reasonably necessary to provide you with the required services and assistance. Providing us with the requested information is voluntary. If you choose not to provide us with the requested information, we may not be able to process your application.
How you can access and update your information
You may ask for access to the information we hold about you at any time and request to update, correct or amend your personal information by calling 13 77 88.
